- Inform the hospital MLO and record the MLC number as your hospital policy requires.
- Write exact times, history in the patient’s own words, and every injury with size and site.
- Preserve evidence: clothing in paper bags, labelled samples, chain of custody.
- Do not give opinions beyond your findings.
- Sexual assault: female doctor, written consent, no two-finger test (illegal).
- Keep notes legible, signed and timed — they may go to court.
