Dengue in pregnancy
Seasonal outbreaks in Karachi, Lahore, Rawalpindi and Peshawar.
Source: WHO · national dengue guidanceWarning signs: abdominal pain, persistent vomiting, fluid build-up, mucosal bleeding, lethargy, liver enlargement, rising haematocrit with falling platelets.
- Pregnancy dilutes the blood, so follow serial haematocrits rather than one value.
- Calculate fluids on pre-pregnancy weight; careful crystalloids.
- Paracetamol only — no NSAIDs or aspirin.
- No prophylactic platelets if not bleeding; keep above 50,000 for birth or surgery.
- Avoid induction or CS during the critical leak phase unless life-saving.
- High PPH risk: deliver in a unit with blood. Watch the baby if infected near term.
