| Mild | Bloating, mild pain, ovaries <8 cm. |

| Moderate | Nausea/vomiting, ascites on scan, ovaries 8–12 cm. |
| Severe | Clinical ascites, oliguria, Hct >0.45, Na <135, albumin <35. |
| Critical | Tense ascites, Hct >0.55, WCC >25, anuria, VTE, ARDS. |
LMWH prophylaxis; avoid diuretics; paracentesis if needed.
